There are two levels of membership in CHeS: Core and Affiliate. A Core Membership is available to any group purchasing organization (GPO) or e-commerce company that has at least 50% of its business in the health care industry. Core Membership dues are $36,000 annually. An Affiliate membership is open to all organizations except GPOs, which can only join at the Core Membership level.

Please complete, sign and submit this application to CHeS, along with the appropriate membership fee (as detailed below). Membership rights and privileges will not commence until payment in full or the first month’s installment of Core Member’s annual membership dues has been received by CHeS.

The term of each membership is one year from the date received of the completed application and payment. Your membership shall automatically renew for successive one-year terms at the same membership level and at the then-current annual membership dues, subject to the terms of CHeS’ by-laws. CHeS will notify you sixty (60) days in advance of the end of each annual term regarding your renewal and any increase or decrease in annual membership dues. You may elect not to renew your membership by notifying CHeS at least thirty (30) days prior to the end of the then current annual term.

Payment Options:
Core Members have the option of paying the full annual dues at the beginning of the membership year, or may pay in equal monthly installments (based on current annual membership dues, this would equal $3,000 per month). Applicants for Core Members must enclose either the entire annual dues or the first month's installment (currently $3,000) along with the application. Core members who opt for paying their dues in monthly installments, agree to make such payments by the fifth (5th) day of each month to avoid a late fee/finance charge at an annualized rate of 12%. Affiliate members must submit the full annual dues along with the completed application.
